What Is Needed Prior To Scheduling My First Appointment With Dr. Cooper
Prior to contacting our office for your initial appointment with Dr. Cooper, the following information will be needed:
- Patient Name, Date of Birth, Address, Telephone# & Email Address
Name of your dental insurance - Click Here To See if Your Insurance is in Network - Dental Insurance Subscriber/Member ID#, Group#, Payer ID# and Customer Service Telephone# located on your dental insurance card.
- Feel free to contact our office directly at (336) 525-9581 with the requested information and/or Email the requested information and a screenshot of your insurance card [front/back] to us at smile@cooperdentistgso.com
- If you have been seen by another dental practice within the 1 ½ years, please contact your previous dental practice to request your most recent images be sent to us via Email at smile@cooperdentistgso.com
- If you have not been seen by another dentist within this timeframe, please kindly let us know when we speak with you.
- Once our office has received the requested information and your policy has been verified, we will proceed in contacting you to schedule your New Patient/Continuing Care dental appointment with our practice. If you do not receive a response within 3 days, please contact the office at (336) 525-9581.
- This appointment normally takes about 1 ½ hours to complete.
